Fieldwork Educator Forms
The AOTA Fieldwork Data Form summarizes and connects information in the curriculum design of OT or OTA program with the level II fieldwork component. This form is filled out by the student and shared with the clinical instructor at the end of the Level II Fieldwork experience.
Student Evaluation of the Fieldwork Experience (SEFWE) Form
The Clinical Instructor Survey Form is a tracking tool used to identify the credentialing and experience of Level II fieldwork clinical instructors. This form is filled out by level II fieldwork clinical instructors and mailed to UB.
Student Evaluation of the Fieldwork Experience (SEFWE) Form
Student Evaluation of the Fieldwork Experience Form is used by the student to evaluate the level II fieldwork experience. This form is filled out by the student and shared with the clinical instructor and academic fieldwork coordinator.
Level II Fieldwork A Student Request Form
The Level II A Fieldwork Site Request form is used by students to request level II fieldwork sites. This form is filled out by the students and returned to the academic fieldwork coordinator at UB.
Level II Fieldwork B Student Request Form
The Level II B Fieldwork Site Request form is used by students to request level II fieldwork sites. This form is filled out by the students and returned to the academic fieldwork coordinator at UB.
Level II Fieldwork Student Progress Reports
The Level II Fieldwork Student Progress Report form is a series of student progress reports that students will complete and turn in to the academic fieldwork coordinator every 2 weeks.
