Any form that requires a signature and date should be hand signed or a digitally verified signature, and not typed.<\/p>\n<p>Documents are either pdf, doc, or excel files. To fill in pdf format documents, you will need Adobe Acrobat. All UB public computing site workstations have this ability, or you can visit the Tech Squad on Silverman 3rd floor for assistance, email <a href=\"mailto:ubithelp@buffalo.edu\" target=\"_blank\" rel=\"noopener\">ubithelp@buffalo.edu<\/a>, or call 716-645-3542.<\/p>\n<h2>Frequently Used Forms<\/h2>\n<ul>\n<li>Requisition Request Form (for requesting money): <a href=\"https:\/\/myubcard.com\/students\/requisition\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/myubcard.com\/students\/requisition<\/a><\/li>\n<li><a href=\"https:\/\/www.buffalo.edu\/administrative-services\/forms-catalog\/facilities\/on-campus-food-event.html\" target=\"_blank\" rel=\"noopener\">Food Permit Application<\/a>&#8211; Any food served on campus that is not ordered through Three Pillars Catering will require an approved food permit.\u00a0 Please note that any food permit application submitted with less than 14 days notice is being denied. EH&amp;S will no longer issue warnings.<\/li>\n<li><a href=\"https:\/\/www.buffalo.edu\/administrative-services\/policy1\/ub-policy-lib\/allowable-use-funds.html\" target=\"_blank\" rel=\"noopener\">University Alcohol Policy<\/a>\n<ul>\n<li>The new campus policy states &#8220;When alcohol is provided at a hosted event, it must be served by a provider licensed to serve alcohol. Proof of use of a licensed provider is required at the time of approval; exceptions are not allowed and will not be processed when alcohol is served without a licensed
