Policy Submittal Form for Global Database on Local Food Policies

  • Provide the official title or name of the local/regional policy (e.g. Local Food Procurement Guidelines, Local Law 50).
  • Provide the year in which the policy adopted or passed.
  • Identify the language in which the text of the policy was written by selecting one of the choices in the menu. If the language is not listed, select "other" and specify in the additional box.
  • Select the food system sector addressed by the policy. You may choose multiple sectors, if applicable.
  • Select the type of policy from the menu below.
  • Select a few keywords that best describe the policy. Select between 5-12 keywords.
    If a keyword is not listed, select "other" and specify in the additional box.
    • Identify the country in which the jurisdiction which adopted or passed the policy is located. Choose from the menu. If the country is not listed, select "other" and specify in the additional box.
    • Provide the name of the state (or, sub-national area) in which the jurisdiction adopting the policy is located.
    • Provide the name of the district or county within which the jurisdiction that passed this policy is located (if applicable).
    • Provide the name of the local government jurisdiction for which the policy was adopted or passed.
    • Select all that apply. If the level of government is not listed, select other and input into the additional box.
    • Provide the most recent population estimate for the jurisdiction, and the year of data. (Separate information with a semi-colon; for example, Population 200000; 2010)
    • Select all that apply.
    • List the name of the government entity that officially adopted the policy (e.g. Accra Metropolitan District). If multiple entities were involved in adopting the policy, list all; separate multiple entities with a semi-colon.
    • List the name(s) of the government or non-government entities responsible for implementing/enforcing the policy. Separate multiple entities with a comma.
    • List the name(s) of the government or non-governmental entities responsible for supporting the lead entity or entities in the implementation/enforcement of the policy. Separate multiple entries with a comma.
    • Enter the amount of funds required for the development and/or implementation of the policy (if available or relevant). Specify currency, please.
    • List the government and non-governmental entities that provided funding for the development or implementation of the policy. Separate multiple entities with a comma.
    • List or summarize the major outcome(s) of the policy. Separate mutiple outcomes with a semi-colon.
    • Upload a PDF version of the policy in its original language.

Indicate the following information: brief description of policy, name and location of adopting sub-national government, policy document (if available), and contact information for an individual who can answer additional questions about the policy.
